Over the past several years, Dalton Associates has been honoured to develop strong partnerships with Indigenous communities and regional councils in northern Ontario to support the development of regional and community-based mental health and wellness programs that are tailored to the unique needs of the region/communities. We use a team approach that assigns consistent, Masters-level registered mental health providers per community, who have cultural expertise and training. Our service approach, supported by our Indigenous Community Advisory Team and the First Nations Wellness framework, is based upon the principles of:

  • Shared care, equity in voice and choice, and representation among stakeholders
  • Integrated approaches to care that support whole-person wellness

  • Focus on resilience, empowerment, and capacity building
  • Culturally safe and attuned services

Our organization is launching, in partnership with indigenous organizations and Regional Health Authorities in Ontario’s north, a program that will provide centralized intake, service navigation, and mental health supports to 49 northern indigenous communities.

At the cornerstone of this program is our team of Wellness Navigators. The Wellness Navigators connect with clients (members of northern indigenous communities) via phone, email, text or chat, and respond to client-related requests (such as registering for services, discussing service options, requesting information, etc.) and oversee the client’s case from beginning to end, including coordination of appropriate support, information management related to the case, resolving client concerns and facilitating relationships between the client and appropriate service providers/partners.

This Wellness Navigator role is primarily for weekend (8am – 4pm, or 4pm – 12am ET) and holiday shifts, with occasional weekday evening shifts (4pm – 12am ET). The Wellness Navigator will work remotely.

Qualifications:

  • Bachelor’s degree or equivalent in relevant field preferred (i.e., Psychology, Social Work, Indigenous Services, Traditional Counselling, Healing and experience in a Mental Health-related field); Registered Nurse with Mental Health specialty; Minimum of a 2-year relevant college diploma or equivalent academic and professional experience preferred (i.e., Social Service Work, Health-related field, Healthcare Administration, etc.)
  • Professional experience in Psychology, Social Work/Social Services, Health, Healthcare Administration an asset
  • Crisis/distress/de-escalation training and experience
  • Detail oriented
  • Excellent organizational and time management skills
  • Excellent verbal, written and interpersonal communication skills
  • Exceptional client service skills
  • Ability to effectively and efficiently problem solve and multi-task
  • Able to work independently and as part of a team
  • Demonstrated high computer proficiency, including MS Office (Word, Excel, Outlook, and PowerPoint)

Key Responsibilities:

  • Partner with other Wellness Navigators to deliver exceptional client service
  • Act as a liaison between clients, their treatment team, and other health agencies
  • Build relationships and trust with the clients to ensure that clients are receiving proper care and treatment in the services they require, and must maintain confidentiality at times.
  • Perform client Triage/intakes and assessments to determine needs
  • Conduct brief telephonic counselling support, as appropriate
  • Facilitate warm transfers of callers in distress to crisis services or to on-call counsellors
  • Provide tailored support for clients throughout their lifecycle with Dalton Associates, including systems navigation, care coordination, communication, and collaboration with the client’s circle of care to ensure best outcomes for client wellness
  • Maintain and update client records in accordance with practice standards
  • Perform regular client file audits to consistently enable excellence in record management standards
